Short answer: preserve the live AF50, then build to the current regulations
Download AF50 from the live WORC Immigration Forms inventory on the filing date, preserve the exact PDF and hash, and compare every answer with section 50, Schedule 5 Part 6, the certificate, current fee materials, and written WORC advice. The live form says filing is between 1 and 31 December, or when the prescribed annual fee is paid on the anniversary of a granted right to work. It also directs the holder to email the completed declaration with proof of CI$500 payment to AnnualDeclarationSubmissions@gov.ky. WORC's 7 August 2026 reminder additionally says declarations may be submitted at Apollo House West and that anniversary or December timing depends on the facility held. Confirm the exact route before signing or paying.

Record the live-form version and the replaced-link problem
On 25 August 2026, the live Annual Declarations tab linked AF50 to a URL ending 2026-06-003f. That PDF is internally marked WORC/RBC DEC (2026/05) AF50 and differs from the earlier 2026-06-002f file cited by the R50 application page. Preserve the current 003f file rather than relying on an older direct URL. Recheck immediately before oath, payment, and submission because a replacement PDF may resolve or change the current conflict.
- Save the unedited live PDF and record the source page, link, download date, page count, internal code, and SHA-256 hash.
- Do not treat a similar filename or successful PDF response as proof that it is still the inventory-selected form.
- Keep the earlier 002f file only as version-history evidence, not as the filing form.
- If WORC replaces AF50, compare every statement, field, footnote, instruction, and warning before carrying answers forward.
Build a form-to-regulation reconciliation before answering
The current regulations prescribe a broader Part 6 declaration than the live one-page AF50 displays. The regulations separate the owner-or-senior-management route from cumulative premises and employee conditions, require the business to still exist, extend health-insurance statements to a spouse or civil partner and dependent children, require change reporting for child dependants, marital/civil-partnership status and occupation, update several sensitive definitions, and require dependant details. Prepare a line-by-line reconciliation and obtain written instructions on how the missing or differently worded particulars should be supplied.

Identity, certificate, prior declaration, and review period
AF50 asks for the certificate name, date and country of birth, and certificate date, then measures facts from grant or the last annual declaration, whichever is later. Build a one-page chronology identifying the operative certificate, any right-to-work variation, prior AF50, dependant certificates, notified changes, and the exact period under review.
- Reconcile certificate name with passport and any legal-name change evidence.
- Record certificate number, grant date, approved business or businesses, route, occupation, conditions, dependants, and variations.
- Retain the last submitted declaration, attachment set, payment, delivery proof, responses, and any later correction.
- Separate the declaration review period from the preceding-calendar-year employee and physical-presence tests.
Owner route or senior-management route evidence
Current Part 6 asks the holder to declare either qualifying direct or indirect ownership in an approved category of business or that the Director of WORC was satisfied that the holder is employed in a senior-management capacity. Do not rebuild the original application from memory. Reconcile the current legal, beneficial, economic, employment, governance, and certificate records to the exact route on which the certificate depends.

Commercial premises, business existence, licences, and operating permission
Part 6 requires the relied-on business to maintain a physical Cayman presence through purchased or leased commercial real estate and separately asks that the business still exists. The live form also asks about lost operating/work permission and revoked licences. Reconcile the entity, premises, activity, TBL/LCCL and sector permissions, lease or ownership, occupancy, utilities, insurance, planning position where relevant, and actual trading evidence.
- Identify every relied-on business by legal name, registration number, activity, premises, licence, and certificate connection.
- Distinguish revocation, non-renewal, expiry, pending renewal, variation, suspension, closure, dormancy, relocation, and temporary interruption.
- Retain current lease or title evidence, landlord/strata permission, floor plan, utilities, equipment, signage, staff use, and dated operating records.
- Do not treat a registered office, mail address, licence payment, company existence, or lease alone as proof of current substantial business presence.
Four full-time employees and nine-month evidence
The current regulations ask for at least four full-time employees who are legally and ordinarily resident in Cayman for at least nine months in the past calendar year. Build an employee-by-employee schedule and reconcile contracts, start/end dates, hours, payroll, bank payments, work permissions, residence evidence, health insurance, pension, leave, transfers, and employing entity. This page does not decide how vacancies, contractors, related-company staff, leave, remote work, or partial periods are counted.

Preceding-calendar-year physical presence
Current Part 6 asks the holder to declare at least ninety aggregate days of physical presence in Cayman in the calendar year preceding the declaration. Build the count from passports, itineraries, boarding passes, travel-account history, and available immigration records. Do not decide partial-day, emergency, medical-travel, detention, record-gap, or disputed-movement treatment without advice.
- Fix the exact preceding calendar year before calculating.
- Use a dated arrival/departure worksheet tied to primary evidence.
- Reconcile passports replaced mid-year, multi-leg travel, cancellations, and missing records.
- Escalate a shortfall, uncertain day, exceptional fact, or conflict before the oath is sworn.
Insurance, dependants, and required change reporting
The current regulations require adequate health insurance for the holder, spouse or civil partner, and dependent children, and require declarations about changes in child dependants, marital or civil-partnership status, and occupation, including whether the Director was informed under section 50(9). They also require details for accompanying and non-accompanying dependants. The live one-page AF50 does not show those current particulars. Build the full household and notification file and obtain instructions for any supplemental schedule.

Review every sensitive declaration against current wording
The live form and current Part 6 differ across whose convictions and health facts are covered, traffic-offence treatment, communicable-disease wording, mental-health terminology, and spouse/civil-partner scope. Current Part 6 uses the Mental Health Act (2023 Revision) language and includes the holder's spouse or civil partner in several statements. Read the current regulation and filing form exactly, preserve primary records, minimize sensitive data, avoid diagnosis or stigma, and obtain legal advice where meaning or disclosure is uncertain.
- Do not paraphrase conviction, political/racism, public-funds, communicable-disease, prostitution-related, mental-health, or unauthorized-dependant wording from memory.
- Separate a factual record from a legal classification and from the decision about what must accompany the declaration.
- Do not assume the live form's traffic-offence exception or dependant scope overrides the current prescribed particulars.
- Keep medical, child, family, criminal and immigration records access-controlled and disclose only what current instructions require.
Written explanation, oath, payment, submission, and closeout
The live AF50 says a holder unable to make any of its thirteen statements or sub-statements without making a false statement must explain in writing without delay. Current Part 6 says a written explanation must be included in the declaration when any prescribed declaration cannot truthfully be made. Treat those instructions together: stop before signature, prepare a factual chronology and evidence index, obtain advice on the required wording and channel, and preserve the final sworn package. The live form provides for oath in Cayman before a Justice of the Peace or Notary Public, with signature and seal.
- Do not leave a known conflict for correction after signature or omit a current Part 6 particular because it is absent from the live page.
- Confirm whether the explanation belongs in, with, and/or separately before the declaration and how receipt will be proved.
- Retain the exact signed and sworn AF50, supplemental schedule, explanation, payment proof, attachment index, email delivery or office receipt, and immutable copy.
- Treat the false-statement and revocation warnings as reasons for careful review, not as a prediction that every error automatically produces a particular consequence.
- Calendar the next declaration and earlier ownership, role, premises, staffing, licence, travel, insurance, dependant, marital-status and occupation controls.
