Short answer: this is a declaration, not a tax-residence application
The current DITC individual self-certification form asks an individual account holder to identify themselves, state every jurisdiction in which they are tax resident, provide the relevant tax identification number or permitted explanation, and sign or positively affirm the declaration. It does not decide your tax residence for you. Moving to Cayman, holding a work permit, receiving a residency document, or changing an address does not by itself answer whether another jurisdiction still treats you as tax resident.
- Use the financial institution's current form and instructions; it may adapt the DITC template or request additional evidence.
- Do not copy a spouse, employer, advisor, company, or previous bank answer unless the same facts and legal analysis genuinely apply.
- List all relevant tax residences rather than forcing a single-country answer where the facts may support more than one.
- Get jurisdiction-specific tax advice before signing when residence, treaty, departure, citizenship, domicile, payroll, company, trust, pension, or investment facts are unclear.
Confirm who is completing the form
The DITC individual form is designed for an individual account holder, sole trader, or sole proprietor. Joint or multiple account holders use a separate form for each individual. A guardian, attorney, nominee, custodian, or other authorised person completing the form for someone else should identify the capacity in which they sign and keep the authority document with the account file.

Build the identity and address file first
A self-certification is easier to review when its basic identity fields match the institution's customer-due-diligence file. Resolve spelling, name, date-of-birth, address, citizenship, and document-version differences before the form is signed.

List every jurisdiction of tax residence
Tax residence is determined under the rules of each relevant jurisdiction, not by the label on a Cayman immigration document. The current DITC form warns that special circumstances can make a person tax resident elsewhere or in more than one jurisdiction. The OECD's CRS guidance likewise expects all tax residences to be disclosed in the self-certification.
- Start with every country connected to citizenship, recent residence, homes, family, employment, directorships, business activity, investment accounts, pensions, or extended day counts.
- Record the date facts changed, such as departure, Cayman arrival, job start, home availability, property sale or rental, spouse move, or work-pattern change.
- Ask the relevant home-country advisor to document the conclusion where residence is not obvious.
- Do not assume a treaty tie-breaker, work permit, residency right, or Cayman address means only one jurisdiction should appear on a CRS self-certification.
- Keep the advice note and the facts used to reach it; the bank form should not be the only record of the decision.
Prepare TINs and explanations before submission
The form asks for the tax identification number or functional equivalent for each stated tax residence, subject to the form's permitted reason codes and explanation fields. Do not leave the field blank merely because a number is hard to locate or a country uses a different label.

Handle US-person and FATCA questions separately
The DITC individual form says a US citizen or US tax resident should identify US tax residence and may also need an IRS Form W-9. FATCA questions should not be collapsed into a generic CRS answer. US citizenship, green-card history, substantial-presence facts, expatriation, treaty positions, and entity or trust connections can require specialist analysis.
- Ask which FATCA form the institution requires and whether a W-9, waiver, or additional US indicia evidence is needed.
- Make the US-person answer consistent across Cayman and overseas banks, brokerages, pensions, insurers, trusts, payroll, and tax returns.
- Do not use a foreign address or non-US passport as proof that US tax status ended.
- Escalate conflicting citizenship, birthplace, green-card, tax-return, TIN, address, phone, or standing-instruction facts before signing.
- Keep the signed FATCA form, supporting advice, and institution response beside the CRS self-certification.
Reconcile the form with the rest of the move file
A self-certification can be internally complete yet still look unreasonable when compared with the institution's other records. Before submission, reconcile the form with the passport, permit, employer letter, payroll, address, overseas tax, company, trust, pension, and source-of-funds files.

Track changes in circumstances
The current form remains usable only while the information is correct and complete. It instructs the signer to notify the institution and provide an updated self-certification when a change in circumstances makes the existing information inaccurate or incomplete. Treat this as an account-maintenance obligation, not a one-time onboarding task.

Use one cross-institution tracker
Banks, brokerages, pensions, insurers, trustees, corporate-service providers, and other financial institutions may request their own form, portal affirmation, supporting documents, or remediation response. One tracker prevents different versions of the same facts from drifting across providers.

Questions to ask before signing
Use precise questions when the institution's form, the DITC template, and your personal facts do not line up neatly.
- Is this the current individual form for this account, or do you require an institution-specific version?
- Which people need separate forms for a joint, minor, trust-connected, company-connected, pension, insurance, or investment account?
- Which tax residences and TINs does the institution expect based on the records already held?
- What explanation or evidence is accepted when a jurisdiction does not issue a TIN or a TIN is being obtained?
- Which US indicia or FATCA documents require a separate response?
- Which signature, portal affirmation, guardian, power-of-attorney, certification, or secure-upload rules apply?
- What events must be reported as a change in circumstances, through which channel, and within what institution-set timeline?
- Who should resolve a tax-residence question before the form is signed: the institution, home-country tax advisor, US tax advisor, Cayman accountant, lawyer, trustee, or another specialist?
